If the applicant is a limited company, please provide a copy of the Memorandum and Articles of Association and the original Certificate of Incorporation (or a copy certified by your accountant.)
Confirmation of identity is required, e.g. passport or a full photo driving licence and two confirmations of address by way of a utility bill etc (for each party to an application, including all directors of limited companies).
All joint applicants and all partners in partnerships must sign this Application Form and all subsequent legal documentation. If the applicant is a limited company, all documentation must be signed by the appropriate authorised official(s) of the company.
